The middle cerebral artery may be occluded by thrombus or embolus. The clinical presentation depends upon the site of the occlusion and whether or not the dominant or non-dominant hemisphere is affected. There are often profound neuropsychological deficits.

Transient ischaemic attacks in the middle cerebral artery territory characteristically produces a transient hemiparesis in association with dysphasia.
