Hyperextension may tear the anterior longitudinal ligament. There is no fracture but an extension X-ray film shows a gap between the two vertebral bodies. This is most common in the cervical spine.
Neurological damage is variable. The injury is stable in flexion and is treated using a cervical collar for 6 weeks.
