Incubation period is usually 1 to 3 weeks but may be months. Symptoms and signs include:

  • fever, sweating, malaise and aching are typical in over 90%.
  • anorexia and weight loss occur in 20-50%
  • epididymo-orchitis and scrotal pain occur in men.
  • chronic fatigue and depression may occur.
  • lymphadenopathy, splenomegaly (often tender), hepatomegaly in 50%.
  • arthritis, sacro-ileitis and bursitis
  • iritis
  • cough - if infection is by inhalation
  • non-specific rashes
  • vomiting

Chronic illness may follow an acute attack, and is characteristed by recurrent attacks of fever with fatigue and headache as well as limb and back pain. Liver and splenic calcification may occur.
