Septic abortion

A septic abortion is a spontaneous abortion associated with an infected uterine cavity. It most often follows an incomplete abortion which has become infected with vaginal or bowel organisms. An attempt at illegal abortion should be suspected. There is a risk of spread of infection. Generalised peritonitis, septicaemia or renal failure may result.

Clinical presentation:

  • febrile and very ill
  • often there is an offensive vaginal discharge

Diagnosis:

  • HVS
  • MSU and blood cultures
  • kidney function, liver function and coagulation status

Management:

  • resuscitate
  • administer antibiotics
  • evacuate
  • closely monitor progress
